The Rural Health Transformation Program (RHTP) is a landmark federal investment to strengthen healthcare access, quality, and outcomes for rural communities across Alaska. RHTP will create lasting, system-wide change in how healthcare is delivered across the state.
Alaska is among the top recipients of RHTP funding nationwide, reflecting the state’s vast geography, remote communities, and longstanding challenges in healthcare access. In its first year, Alaska received $272 million to invest in transforming its rural healthcare ecosystem.

In Alaska & Beyond
Urban projects count when they strengthen rural health
Organizations anywhere in Alaska, as well as those outside of Alaska, are eligible to apply.
Although RHTP is designed to improve health in rural regions, Alaska’s health system is deeply interconnected. Projects based in urban areas that strengthen rural health outcomes may also be eligible.

About this Program
The Rural Health Transformation Program is a statewide initiative funded through the Alaska Department of Health and the Centers for Medicare & Medicaid Services (CMS) to strengthen rural healthcare delivery across Alaska.
The Alaska Community Foundation serves as the contracted grant administrator for RHTP, responsible for application intake, review coordination, award administration, and ongoing program operations, in accordance with program guidelines established by the Alaska Department of Health. Final award determinations rest solely with the Alaska Department of Health.
