Hilcorp Contingent Beneficiary Form

As the advisor of my fund, I recommend that the President of the Alaska Community Foundation (ACF), as authorized by the Governing Board, consider the following nonprofit organization(s) to be listed as my contingent beneficiary in the event of my death. I understand that ACF will grant out the balance in my fund to this organization if no other request is submitted by me or the additional advisor listed on my fund within 60 days from my death or termination in order to close my fund. If no gifts from me personally have been made into my fund, the balance will be granted to the Hilcorp Donor Advised Fund.

Employees can designate up to three Section 501(c)(3) nonprofit organizations as beneficiaries and allocate a percentage of their remaining fund balance for each organization. Total percentages should equal 100%.
